How To Download Form Submission Reports

A guide on the CSV, XLSX and PDF reports you can download for form submissions

Download CSV or XLSX (Spreadsheet) Reports

To download a CSV or XLSX report of all form submissions received, go to your form library, and click on the template to navigate in to view the form submissions.

To download a spreadsheet report of all form submissions received:

  1. Go to your Form Library.

  2. Click on a Form template to view the form submissions.

  3. Click on the Export button, and select either the .csv or .xlsx options.

  • Your export will include only the submissions that match the filters you’ve applied (e.g., by date, user, job).

  • A ZIP file will download containing one or more spreadsheet files.

  • You may see multiple files if you have previously published different versions of the template.

  • Currently this report does not support display signatures or images - they will be shown as links that can be opened up to view the signature or image on a web browser.

Dynamic Matrix Export Formatting

Forms that include matrix-style questions (such as Calculation Formulas) now generate separate tabs (worksheets) within your exported file.

This preserves the structure of your rows and columns exactly as they appear in the form, while clearly associating with the unique form submission ID, so that you know which submission it is from.

This makes your data immediately usable in Excel, Google Sheets, or BI tools.

Download a PDF

From the form submissions view, you can also download a single PDF of the form submissions by selecting the download button on the form submission row.

Signatures, images and any dynamic matrix used are supported in PDF downloads.
